When printing, if you want to be able to send via mail without having to pay postage, choose properties and find where it says *'print on both sides' and check this box. If you do this, the brochure will print out in its original format. If you do not do this, you will have to fill the application out and mail it in an envelope.

NMB Policy Holder's Address Change Form

NMB Members can now change their address 24 hours a day, 7 days a week. Access the form by clicking on either of the 'NMB Policy Holder's Address Change Form' links. Fill the form out and click submit. If succesful the page will be reloaded with the 'Submission Successful' red label. Print for your own records and your **done!
